Planning Your First Visit

When scheduling a new patient appointment, keep in mind that we will require your arrival time to be 30 minutes prior to your appointment tom complete the new patient registration properly. If you are unable to arrive 30 minutes before your appointment, you may print a copy of our New Patient Registration (link to documents) packet, and fill it out ahead of time. Also, keep in mind the registration forms will need to be filled out completely before your appointment time whether you do it at home or in the office – whatever is most convenient for you.

Below you can click the button to access and print the New Patient Registration documents.

If you were recently in the Emergency Room, or Urgent Care, we will need the entire medical record (NOT discharge summary) prior to your visit to the clinic. Please come into the clinic or into the facility you were seen at, and sign a ‘Medical Release’ form to ensure we receive your records.

If you are coming in for a chronic condition follow up, the medical providers are requesting that your bring your current medication bottle, or a photo of the label.

Yes, our physicians would be happy to see your whole family.

Do you treat children?

Yes, our providers treat children and adolescents. We do not see any children under 4 years old.

Do you offer same day appointments?

We have very limited same-day appointments and they usually book first thing in the morning for urgent matters. We recommend planning ahead and booking your appointment as far in advanced as possible. We do understand that urgent matters come up and we will do our very best to accommodate last-minute appointment requests whenever possible.

What if my doctor is not available to see me?

We encourage patients to see their primary care provider as this is important for continuity of care. If you have an urgent matter and your provider is not available to see you, you may be scheduled with one of our other providers.

How can I reach my provider by phone?

To reach your provider, you may call the office and leave a message with your doctor's medical assistant, you can also text us using the clinic phone number and someone can return your call.. **Please note, we do not diagnose or treat over the phone, so if you have a medical problem, please call the office to schedule an appointment.

What if I need medical care or advice after hours?

Many insurance companies have a 24-hour nurse's line which you can call for medical advice after hours. Please reference your insurance card for this number. If you are in need of medical care, please proceed to the nearest emergency room or urgent care and contact our office the next business day to schedule a follow-up appointment with your provider.
